Dear Occupational Therapist:

We are currently pursuing a Master’s of Occupational Therapy degree at The College of St. Scholastica. We are interested in better understanding the opinions, behaviors, and attitudes of practitioners towards fieldwork education. The results of this study will provide information about fieldwork education in the current healthcare system.

As a member of WOTA, MOTA, AOTA, or alumni of The College of St. Scholastica occupational therapy program, you were selected to be a participant in this study. We are writing to request your participation in this research study, which will involve the completion of an online survey. Completing this brief survey should take no more than 10 minutes of time in your busy schedule.

In order to be eligible to take this survey, you must be a currently practicing OT and have taken at least 1 fieldwork student.

All information collected will be treated with anonymity and summarized in such a way that will ensure your confidentiality. This project has been approved by The College of St. Scholastica Institutional Review Board. Your consent to participate in this study is implied by your completing and returning of the online survey.

To take this survey, please click here. We would appreciate your completed questionnaire by February 20, 2015. If you would like a summary of our findings after the study’s completion, please indicate by providing your email address in the box at the end of the survey. If you include your email address, this information will be attached to your survey responses, therefore, making your responses not anonymous.
